REACH strategies aim to improve health and prevent chronic diseases for Wayne County residents.
Through multisector partnerships, our work is focused on reducing the burden of disease in our communities with unmet needs. The coalition’s efforts will begin in Detroit and later span into its surrounding communities
Our Strategy Areas
The coalition works to address barriers to good health in our communities by focusing on four priorities:
Rooted in Community
After an analysis of Wayne County communities with the greatest prevalence of chronic disease, we will begin working in Detroit later spanning into its surrounding communities.
We’ve established focus groups to ensure we are directly addressing community needs to create the most effective strategies to reduce chronic disease.

As a collaborative effort with community-based organizations and community members, we want to include everyone’s voice in making Wayne county a healthier place to live, learn, work and play.
